What is a Verification of Positive Pregnancy Test and why do I need it?
We offer Verification of Positive Pregnancy Test results based upon the date of your last menstrual period (LMP) and a positive test result with our urine pregnancy test. Some doctors or clinics require proof of a positive pregnancy test before they’ll accept you as a patient or permit you to make an appointment with them regarding your pregnancy.
The Verification of a Positive Pregnancy Test form is the official document that serves as proof of pregnancy for community agencies that provide services such as Supplemental Nutrition Assistance Program (SNAP - food stamps), Temporary Assistance to Needy Families (TANF), or the Women Infants Children (W.I.C.) program. This form is also helpful to document your pregnancy for your employer or health insurance (such as Medicaid for example).
